Rationale

There was a comment from the last ballot that added requirement 11.2.2.2 with concern that some may interpret this as needing to mitigate multiple hazards in general. The intent of this requirement is to require mitigation of multiple hazards only if those hazards are a result of a single common cause failure. Therefore, we are adding clarification to this effect.
